Found it! Audio Technica ATW-T310b. It has a 4 pin connector and it supports 2 channel (one for mic and one for instrument) on the same bodypack. You could use the mic channel for the piezo and the instrument channel for the mag pups. Just need to made a custom cable with 2 1/4 on one side and the mini XLR on the other side.

Two transmitters on one channel equals interference and noise.
