Output 1 not working on Ultra?

I know it sounds a bit wierd, but at the moment only my Output 2 is working on the rear of my Ultra.

I dont know if it is something stupid that Ive done on the control dials. but Im hoping you guys can help me figure out every possible way it could have happened?!
 
QUICKEDIT: The Input is working but only on the RIGHT side not Left/Mono, any Ideas?!?!?

And when I roll my Balance to completely Left in the MIX section of my Layout Menu, nothing comes out, but it will come out of the Right side.

Very odd. or maybe im just an absolute n00b xD
