Output 1 not working on Ultra?

I know it sounds a bit wierd, but at the moment only my Output 2 is working on the rear of my Ultra. I dont know if it is something stupid that Ive done on the control dials. but Im hoping you guys can help me figure out every possible way it could have happened?!
