USB Audio Stopped Working

It's working now. I'm not 100% sure what was the problem. I have tried all the settings combinations a few times. The last move that made the difference was switching the USB output mapping. There are two choices:
1/2 -> OUT1, 3/4 -> OUT2
1/2 -> OUT2, 3/4 -> OUT1

I changed it to the first option then was able to hear. I had tried that before. I don't know why it worked this time. I'm just glad it's fixed. It's not fun chasing these phantom problems. I wish it were simpler.
