MFC boots up in FW Update mode, won't exit...???

For some reason, my MFC Mk.III started booting up in FW Update mode everytime I turn it on, and it won't exit to other modes.

It already had the latest FW.

Anyone else have this happen or maybe have any suggestions on how to fix it?

Have you changed anything; mine started doing that when I added an external foot switch. Had to unplug it during boot up.
 
I've narrowed it down to the external switch 1/2 jack... thought it had been sorted, but that one is still causing this.

It's set up for toggle, cc# 18 to behave as a "wild card" switch. Same for a second controller in the 3/4 jack, but momentary cc#19... no problem with it.

Switches 2 & 4 are turned off, only 1 & 3 active.
 
I think its the actual switch type. e.g. with my Boss FS7 plugged into External 1/2, no issues. If I plug my FS6 into Ext 1/2 and then boot up it puts the MFC into firmware update mode.
 
This particular Mission pedal offers a reverse phase for the external switch (I think)... could that possibly be what's causing the problem?
