Low output on OUT 2

I've set everything the same as OUT 1 but the output on OUT 2 is very low.
When switching the cables on the patch panel I use to connect OUT2 to the patch panel to OUT 1 the output is OK
So there's no problem with the cables.
I think I checked everything but I can't find the fault.
Any idea what might be wrong.
I tried this in the past and all was working fine.

I tried OUT3, also very low output unless pulling out the jack a little bit (tried both mono and stereo cables) and there the Boost/Pad doesn't seem to work.
 
So you did confirm that Out2 is at +4dB (assuming you have Out1 at +4dB), and the physical volume control on the face of the AF3 is rotated to the same relative position? Just a couple of the usual gotcha’s. Best of luck.
 
Output 3 and 4 will be lower than 1 and 2 because it's intended for unity gain with outboard gear when maxed out on then front panel. Still doesn't rule anything out as being wrong, but don't expect those ever match 1 and 2
 
Compare the Level setting in your Output 1 and 2 blocks. Are they the same as well?

If so, I would open a support case.
