I have the same issue. Never quite gets to 100%. I'm not using it as a whammy or anything that I'd need the extra 0.04 units, but it has puzzled me for a while. I'm using an FC-6, with the expression pedal plugged directly into the FC-6. I recently changed the vol pedal to be a global vol pedal so I don't see it or experience it like I did before.
Oh also using Moog EP-3 pedal(s). They're real inexpensive $49... but very usable.
As I understand it, the entire point of running calibration is to take that stuff out of the equation. As I read this I wonder if it has anything to do with the tolerance of the potentiometer used in each pedal. Could a +/- 10% pot inaccuracy (and/or dirty jack and/or dodgy cable and/or… what have you) add up to the anomaly you’re experiencing?
Same problem here with both my Axe FX III’s and EV-1’s.. I just use the Offset settingFWIW, I am having the exact same problem. Max gets to 9.92 (after calibration)
Ok... So, I found the fix was in 1.11:So I was on 1.09. I just updated the firmware. I'm on 1.12 now.
Is this a bug? If this is a bug would it be with the FC12 or Axe Fx?
I've tried 2 expression pedals and both only get to 9.45

Note that it says you must re-calibrate after installing it.
So, you might want to do that.
I'm going to try that on mine later tonight.
See my previous post:Shouldn’t the op already have the fix since he said he updated to 1.12?

I literally just finished and can confirm that all 4 pedals now reach a full 100% at toe down!Ahh, gotcha……did it work for you?
