Antenna combiners - are they a necessity?

Now we’re using two Shure PSM300s for IEMs, and plan to expand to four of those mounted in a rack. Does anybody use more than two transmitters in a rack without a combiner? Or if you added one, does it make a meaningful difference?

I have had 4 transmitters in a rack, different brands 2 sennheiser and 2 carvins.. no issues. If you run one of those amplified extender thingys i'm guessing the combiner would be needed to get multiple systems on that extender.
